If your child is between the ages of 18 months and 12 years please follow these steps:
STEP 1: REGISTRATION – PART A
When registering, Parent/Guardian must provide the following:
- Filled out “Registration form A”
- Copy of Child’s Health Card
- Copy of Child’s Immunization Record
STEP 2: REGISTERATION – PART B
You will receive the SCA Children Center Handbook
- Read the Handbook
- Fill out and sign the Parental Agreement “Registration Form B”
